The California Hospice Foundation (CHF)

The majority of Americans say they would prefer to be cared for and die at home and yet only 38% of us receive hospice care.

Most are never referred to hospice or are referred so late in the process that the patient and family cannot fully benefit from the services.

Quality information is essential if individuals and their loved ones are to be prepared for the choices that come with end-of-life. Not knowing about hospice and palliative care means not being able to choose a comprehensive approach to care at one of the most challenging and vulnerable times of our lives.
